YOUR APPOINTMENT
Please take a moment to print and complete the “Patient Forms for First Visit” found on this page prior to your first appointment. Our office staff can instruct you on which forms you will need when you call to schedule your appointment.
Please bring a copy of your current medications including dosages.
Please bring a copy of your surgical/medical history if applicable.
Please arrive 20 minutes before your initial evaluation appointment in order to complete our Automated Check-In and for our staff to copy insurance cards and collect appropriate paperwork.

YOUR APPOINTMENT
We understand that paperwork is no fun and sometimes frustrating. However, as a licensed medical practice, we are required by law to provide patients with specific documents. At Ridgeview Physical Therapy & Wellness Center, we make every attempt to ensure that the process is as easy as possible.
To minimize any delay in initiating your first visit with us, we’ve provided necessary information and documents that require your signature to be completed prior to your first appointment. Please review, complete and bring these forms with you.
Also, please bring a list of current medications and dosages, medical and surgical history. Please remember to arrive 20 minutes prior to your initial evaluation in order to complete additional medical questionnaires that are specific to your condition. Check in for your first appointment will require some data input on a tablet or paper that we will be more than happy to assist you with.
