YOUR APPOINTMENT


Please fill out all online paperwork prior to your appointment. Our office staff can instruct you on how to receive this email when you call to schedule your appointment.

Please bring your Driver’s License, or Real ID and a copy of your insurance card to your first appointment.

Please arrive 15 minutes before your initial evaluation appointment in order to complete our Automated Check-In and for our staff to copy insurance cards and collect appropriate paperwork.

PATIENT FORMS FOR FIRST VISIT


We understand that paperwork is no fun and sometimes frustrating. However, as a licensed medical practice, we are required by law to provide patients with specific documents. At Ridgeview Physical Therapy & Wellness Center, we make every attempt to ensure that the process is as easy as possible.

To minimize any delay in initiating your first visit with us, we’ve provided necessary information and documents that require your signature to be completed prior to your first appointment. Please review and complete these forms.

Also, please bring a list of current medications and dosages, medical and surgical history. Please remember to arrive 15 minutes prior to your initial evaluation in order to complete additional medical questionnaires that are specific to your condition. Check in for your first appointment will require some data input on a tablet or paper that we will be more than happy to assist you with.
